Consent and Practice Policies
How LOGiC Health protects and manages your information

By completing the Health History & Consent form and utilizing LOGiC Health's platform, you acknowledge that you have read, understood, and agree to the following consent and practice policies. These policies govern related telehealth services and the patient-provider relationship.
1. Informed Consent for Telehealth Services
Nature of Telehealth Services:This Consent is intended to inform you of the nature of telehealth services and to obtain your voluntary agreement to receive healthcare services via telehealth from independent licensed healthcare providers (“Providers”) affiliated with a professional medical entity (the “Medical Group”) accessible through the LOGIC Health platform (“Platform”). LOGIC Health is a management services organization (MSO) and technology platform. It does not provide medical care. Providers provide telemedicine services through secure video, audio, and asynchronous messaging platforms. Telehealth involves the use of electronic communications to enable healthcare providers to diagnose, consult, treat, and provide health education using interactive audio, video, or data communications.
Benefits of Telehealth: Telehealth services provide improved access to healthcare, convenience, reduced travel time and costs, and the ability to receive care from your home or preferred location.
Limitations of Telehealth: Telehealth has inherent limitations including the inability to conduct physical examinations, delays in medical evaluation and treatment if there are deficiencies or failures of equipment, potential technology failures, and limitations in emergency situations. Providers will inform you if telehealth is not appropriate for your condition and refer you to in-person care when necessary. In rare instances, security protocols could fail, causing a breach of privacy of personal medical information.
Presence of individuals other than Healthcare Providers: You understand that others may also be present during your telehealth consultation other than your healthcare provider and consulting healthcare provider in order to operate the video equipment. The foregoing individuals will all maintain confidentiality of the information obtained. You further understand that you will be informed of their presence in the consultation and you will have the right to request the following: (1) omit specific details of your medical history/examination that are personally sensitive to you; (2) ask non-medical personnel to leave the telehealth room; and/or (3) terminate the consultation at any time. You also understand that your healthcare information may be shared with other individuals for scheduling and billing purposes.
Alternatives and Questions: You have had alternatives to telehealth explained to you and you are choosing to participate in telehealth consultation. You have also had direct conversations with your healthcare provider in which you have had an opportunity to ask questions in regard to your telehealth services. Your questions have been answered and the risks, benefits and any practical alternatives have been discussed with you in a language you understand.
2. Patient Rights and Responsibilities
Your Rights: You have the right to:
- Receive quality care regardless of the delivery method
- Confidentiality of your medical information in accordance with HIPAA
- Access your medical records
- Refuse telehealth services at any time without affecting your right to future care
- Know the credentials of healthcare providers treating you
Your Responsibilities: You agree to:
- Provide accurate and complete health information
- Be located in a state where Providers are licensed during consultations
- Ensure you have the necessary technology and internet connection
- Follow treatment plans and medication instructions as prescribed
- Report any adverse reactions or concerns promptly
- Maintain appropriate follow-up care, including required lab work
3. Scope of Services
Providers provide telehealth services for hormone replacement therapy, peptide therapy, medical weight loss, sexual wellness, athletic optimization, cognitive function enhancement, and diagnostic testing. Their services include:
- Initial consultations and health assessments
- Treatment plan development and ongoing management
- Prescription services through partner pharmacies
- Laboratory testing coordination
- Follow-up consultations and monitoring
- Medication adjustments as clinically appropriate
4. Provider-Patient Relationship
A valid provider-patient relationship is established upon completion of your initial consultation. Providers are licensed healthcare professionals with extensive training in specialized treatment areas. Your Provider will evaluate your health history, symptoms, and goals to develop an appropriate treatment plan.
Primary Care Coordination:The Providers' services are intended to complement, not replace, your relationship with your primary care physician. You are encouraged to maintain regular care with your primary care provider and inform them of treatments you receive through the Providers.
5. Privacy and Security
LOGiC Health is committed to protecting your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). All telehealth sessions are conducted by Providers using HIPAA-compliant, encrypted platforms. Your medical records are securely stored and accessible only to authorized healthcare personnel involved in your care. For complete details, please review our Privacy Policy.
6. Prescriptions and Medications
Prescription Policy: The Providers may prescribe medications when clinically appropriate. We partner with accredited 503A and 503B compounding pharmacies to provide high-quality medications. Prescriptions will be sent directly to partner pharmacies for fulfillment and delivery to your address.
Controlled Substances: Some treatments, including testosterone replacement therapy, involve controlled substances regulated by the DEA. Current regulations allow for telehealth prescribing of these medications. Prescriptions for controlled substances are subject to specific regulations including refill limitations and periodic follow-up requirements.
Medication Safety: You are responsible for following dosing instructions, storing medications appropriately, and reporting any adverse effects. Due to regulatory requirements, medications cannot be returned once dispensed.
7. Laboratory Testing
Hormone replacement therapy and other treatments typically require laboratory testing for safe and effective treatment. Initial lab work is required before starting treatment, with follow-up testing every 6-12 months or as clinically indicated. We coordinate with LabCorp, Quest Diagnostics, AnyLabTestNow, and mobile phlebotomy services to make testing convenient. Lab fees may be included in your membership tier or billed separately.
8. Emergency Situations
Telehealth Limitations in Emergencies:The Providers' telehealth services are not appropriate for emergency medical situations. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Do not use telehealth services for urgent or life-threatening conditions.
9. Technology Requirements
To participate in telehealth services, you must have access to a device (computer, tablet, or smartphone) with internet connectivity, a camera, and audio capabilities. You are responsible for ensuring your technology is functioning properly. Technical difficulties may require rescheduling of appointments. Neither LOGiC Health nor the Providers are liable for information loss due to technical failures. You also agree to conduct your telehealth consultation in a private space and not to engage in any telehealth services while driving.
10. Appointment and Cancellation Policy
Appointments should be canceled or rescheduled at least 24 hours in advance. Late cancellations or missed appointments may result in fees. You will receive appointment reminders via email and/or SMS.
11. Payment and Billing
The Providers operate on a direct-care model and does not accept insurance. HSA and FSA payments are accepted. Membership fees, consultation fees, and medication costs will be clearly communicated before services are rendered. Payment is due at the time of service unless alternative arrangements have been made.
12. State Licensing and Geographic Restrictions
Providers are licensed in multiple states and provide services nationwide. You must be physically located within a state where your provider is licensed at the time of your telehealth consultation. You confirm that you are located in an eligible state during your consultation.
13. Medical Records
Electronic medical records are maintained for all patients. You have the right to access, review, and request copies of your medical records. Documentation of all telehealth encounters is maintained in accordance with applicable laws and professional standards.
14. Informed Consent for Treatment
Before initiating any treatment, your provider will discuss the risks, benefits, and alternatives with you. Hormone replacement therapy and other treatments carry potential risks and side effects, which will be thoroughly explained. You have the right to ask questions and decline treatment at any time. Your electronic signature on the Health History & Consent form constitutes your informed consent to treatment.
15. Ending Treatment
You may end your treatment at any time. If you choose to terminate treatment, it is encouraged that you discuss your plan to end services with your healthcare provider prior to taking action so that appropriate recommendations and referrals can be provided as needed. LOGiC Health reserves the right to terminate your access to the Platform upon its discretion if you do not comply with the terms of this Consent and Practice Policies form. The Providers reserve the right to terminate your care if at any time, your clinical needs require a level of care not provided by the Providers.
16. Modifications to Policies
LOGiC Health reserves the right to modify these policies at any time. Updated policies will be posted on this page with the effective date. Continued use of the Platform after modifications constitutes acceptance of the updated policies.
17. Acknowledgment and Agreement
By checking the consent box on the Health History & Consent form and providing your electronic signature, you acknowledge that you have read, understood, and agree to these Consent and Practice Policies. You confirm that you have had the opportunity to ask questions, all of your questions have been answered to your satisfaction and the risks, benefits and practical alternatives have been discussed with you in a language that you understand.
